Glaucoma Surgery: Lower Eye Pressure to Protect the Optic Nerve

Glaucoma is a chronic eye condition characterized by increased intraocular pressure, which can damage the optic nerve and lead to progressive vision loss. When eye drops and laser treatments are no longer effective, glaucoma surgery may be necessary.

When Is Glaucoma Surgery Recommended?

  • Eye pressure not controlled despite maximum medical therapy

  • Progressive visual field loss confirmed by exams

The goal of surgery is to promote better aqueous humor drainage in order to permanently lower intraocular pressure.

Before the Procedure

Pre-operative assessments:

  • Intraocular pressure measurement (tonometry)

  • Automated visual field testing

  • OCT of the optic nerve

  • Gonioscopy (assessment of the iridocorneal angle)


The surgical technique is selected based on the glaucoma type: open-angle, angle-closure, secondary, etc.

The Surgical Procedure

Duration: 30 minutes to 1 hour depending on the technique used.

Anesthesia: local or general depending on the case and chosen technique.

Main surgical techniques:

Trabeculectomy: the reference method that creates a drainage pathway for the aqueous humor into a filtration bleb under the conjunctiva.Filtering implants (Ex-Press®): a small device inserted to regulate intraocular pressure, often used in combination with trabeculectomy.Minimally Invasive Glaucoma Surgery (MIGS): such as iStent inject® or Hydrus™, ideal for moderate glaucoma with reduced recovery time.Cyclophotocoagulation: a laser technique that reduces aqueous humor production, used in advanced or refractory cases. The procedure is performed on an outpatient basis or with a short hospital stay, depending on the case.

After the Procedure

Post-operative care:

  • Instillation of anti-inflammatory eye drops

  • Regular checks of eye pressure and healing

  • Light physical activity for 1 to 2 weeks


Goal: to stabilize the disease and slow glaucoma progression.


Recommendations:
  • Do not rub your eyes

  • Avoid dusty or humid environments

  • Follow medical instructions carefully
